Financial Policies

CardioVascular Consultants of Montana is sympathetic to the financial health of our patients and offer the following for your consideration:

Payment Options: Payments can be made at the time of service using cash, check, or credit card (VISA and MasterCard accepted).  We will also accept payment by mail.

Insurance: Our office accepts most health insurance plans.  Our staff will assist with requirements and prior authorization necessary for your insurance provider.  We require that your insurance card be presented at time of check-in with our front office staff.  Patients are responsible for payment for all services rendered that are not covered by your insurance plan.

Cash Pay: Cash patients (when an insurance company is not being billed for services) are required to make a payment at time of service and will receive a discount if services are paid in full at the time of the visit.  If this is not possible, payment plans are also available.  Please contact Heidi, Director of Financial Relations, at 406.325.5560 prior to your visit to discuss your options.

Forms that will require signatures at check-in are posted below. You may download, print, and fill out the appropriate forms prior to your visit, or for your convenience, you can electronically sign the forms at your visit.

Prescription Refills

We strongly encourage patients to contact their pharmacy to send an electronic refill request to us or to fax a refill request to 406.325.5556. This is the quickest way to have your prescription medications refilled.

Prescription renewals by phone are handled Monday through Friday from 8 AM to 5 PM, excluding holidays. Please allow 24-48 hours to accommodate your refill request.

If your prescription is running low, please do NOT wait until you have completely run out of your medicine or the weekend to refill your medication. We understand that unexpected circumstances can occur, but respectfully ask that you take care of these issues accordingly. If you are planning to travel, please make sure to have enough medication to take on your trip.

This is a list of a few motels located within a short drive to our office and to the area hospitals in town. Many of these motels will offer a hospital/medical discount and also transportation if needed, so please ask.
